| Power Announcements - Emergency Notification Form |
This form should be filled out by parents each year and submitted to the MSHS Guidance Department in the MSHS. It allows parents to receive emergency broadcasts (School Closings) |
| Response to Intervention Forms (RtI) |
Student performance is influenced most by the quality of the interventions we deliver and how well we deliver them. The RtI forms should be use for the process. |
| Use of Weight Room @ MSHS |
Community Members need to have the following forms on file with the Superintendent's Offices.

| Assumption of Risk - Walkers |
This form should be submitted to the building secretaries if any community member is looking to walk the hallways in the morning or evenings. |
| Volunteer Form |
If you are planning on volunteering at any school in Cuba-Rushford, please submit this form filled out to the principal of the building you intend to volunteer at for approval. Once the the principal approves you, they will send it to the district office for Board Approval at the next Scheduled Board Meeting. |
